Patient Info
Appointment Information
Patients are seen by appointment during office hours. If you are a new patient, you will receive information and instructions which will allow you to complete the necessary forms ON LINE prior to being seen. Existing patients may also obtain access to the Patient Portal where they may update information, request refills, request appointments, check test results, and view a synopsis of their medical history. Emergency patients will also be seen as needed. If you need to change or cancel an appointment, please call and let us know in advance.
Every effort will be made for you to be seen on time. Should a delay occur, you will be informed of the nature of the delay (i.e. emergency, delivery, etc.), and given an opportunity to reschedule your appointment if waiting is not feasible. We encourage your suggestions in our efforts to improve the attention and care you receive.
Reception Area
Due to the serious risk of childhood illnesses to our pregnant patients and their unborn, we ask that no children between the ages of 1yr. through 12yrs, old accompany you. In an effort to provide a healthy environment, smoking is not permitted throughout the office.
We ask that no food or drink be consumed in the office.
Knowledgeable, Helpful Nurses
The nurses in our office all have extensive obstetrical and gynecological and urogynecological experience. They can handle many of your questions and problems and will also provide patient information materials specific to your situation.
Questions and Telephone Calls
The office telephone number (972) 781-1444 is answered 24 hours a day. The answering service will take your calls before and after office hours. It is important that you indicate the degree of urgency of your call so that the answering service can relay messages appropriately. In most cases, urgent care (including labor) and minor emergency care should be initiated by first calling the office phone number. On occasion, another obstetrician-gynecologist may be taking emergency calls for us and our answering service will direct you accordingly. For nonemergent calls, you may send secure e-mail through the patient portal for your convinence.
Confidentiality Respected
Your medical record is strictly confidential. Your prior written authorization is required for the release of any information. Please notify our office promptly of any changes in your name, address and telephone number.
Beneficial Patient Education
It is important that you learn about and understand your medical condition. Many educational materials including pamphlets and video programs are available on our web site for your use. Questions and discussions are encouraged.
Reasonable Fees
We share your concerns regarding the rising cost of healthcare. The expense of billing adds to your cost of medical care and, therefore, we ask for your payment of co-pays, deductibles and non-covered services at the time services are rendered. We accept cash, checks, Visa and Master Cards. Arrangements will be made for a payment plan when needed. Details on obstetrical and surgical billings will be provided separately.
Simplifying Insurance Payments
Please bring verification of your insurance coverage with you on your first visit, and keep us informed of any changes in your coverage. We will assist you in preparation of insurance claims for obstetrical and surgical services.
Please check the web site prior to your visit to confirm our participation in your insurance program. You will then need to present your insurance identification card to the receptionist when you arrive for your appointment. We will be happy to assist you with your questions.
